Currently there is no UI to change filename format (or any other settings).
Maybe later (not now) I will and a popup with some settings: to disable/enable some features, backup/import the download history.
This userscript's filename format maybe looks a bit weird if you see it the first time, but based on my experience it's the best format, see why: https://github.com/AlttiRi/twitter-click-and-save#filename-format.
In additional, it's the only one reason why I published this userscript — to popularize such file naming approach.
If you really need to change the filename format, as a workaround you can edit one line in the source code. (Note: the change will disappear on the userscript auto update event.)
